Health Force One is passionate about the relationship that we embark upon with our clients. We have a couple of forms to look at and fill out before we begin working with you. 

Informed consent

The purpose of this form is for you to be aware of the specifics of what we are offering to you, and a little bit about our intentions and responsibilities.                                                                                                    

Food and Lifestyle diary

Before our first consultation keep a detailed dairy of 1 week of the foods that you eat and drink, how many minutes of exercise you are getting each day, what your moods are, how you sleep, your level of energy in the morning and evening and how your body is eliminating waste. Do not be concerned about changing your current lifestyle and dietary habits. This is not about judgement, it is about learning, so honestly record your habits throughout the week and have fun with it !!.

Intake information

This form gives us all of the basics, and a starting block to develop a care plan just for you. It also allows us to cut to the chase with your health, and to be sensitive to anything that may take special care.
